Accepted Insurance Plans

Our practice is actively working on credentialing with most major insurance plans and most of the smaller insurance plans. 

If you do not see your insurance listed below, please call the office to speak with one of our knowledgeable staff members about your individual coverage.

If we do not currently accept your insurance, we do have cash pricing and discounts. For questions about pricing and payment plans, please call our office and we will be happy to discuss.

If you have an HMO, you will likely need a referral from your primary care physician. The referral must be in place prior to your first appointment. Referrals can be faxed to us at 603-218-6254.
